1.Be sure to fill out the pre-examination form and measure your body temperature before visiting clinic. (Click here to download the pre-examination form. If you cannot print it, we can give it at the clinic in advance.)

② Before you are called to the examination room, please take off your jacket so that you can easily receive the vaccination.4 Please prepare the accounting in advance. It will be 3000 yen per person.

Accounting methods include cash, credit card (including touch payment), transportation IC, ID, QUICPay, and PayPay. I would appreciate it if you could check the charge and prepare for cashless payment so that you do not get change as much as possible.
